New Patient Information

Welcome to Healthway! Our team of internal medicine doctors and staff are glad you’re here.

If this is your first visit, expect the signing in process to take a little extra time. We’ll need you to fill out some new patient paperwork before we can get started. To speed up this process, you can download the forms below and bring them with you to your first visit.

You’ll also need to bring along your current insurance card, a photo ID and a list of your current medications (you can also bring the medication bottles themselves).

All pediatric patients under the age of 18 must be accompanied by a parent or legal guardian.

Self Pay

For self paying patients, the cost for the first visit is $110. Established patients are charged $75 a visit.

All private and most Medicaid insurances including:

  • Passport

  • WellCare

  • Aetna Better Health

  • Anthem Medicaid

Others:

  • Medicare

  • Tricare
